If you misplaced the prepaid return label for your LifeOnRecord phone rental, you can request a copy here.
Enter your invitation or order number and ZIP code below. If we find a match, we will provide a copy of your return label.
After selecting the button, check your browser's Downloads folder for the PDF. If you choose to email a copy too, please also check your inbox and spam folder.
How To Pack And Return Your Phone
Please include all of the original items in the box:
- phone
- handset
- AC adapter
- battery pack
- batteries
- sign, if you ordered the Communication Package
If any accessories were lost, please use this reimbursement form:
Reimbursement for Lost Accessories
If you no longer have the original packaging, you may use your own box. Please pack the phone securely with enough padding to prevent movement during shipping. Wrap the handset separately from the phone body and make sure it cannot strike the phone if the box is dropped or tossed. Nothing in the box should rattle around.
Once everything is packed, please drop the package off at a USPS location or schedule a USPS pickup:
